To register your child with Cheder Chabad, please complete the following three steps:
You will need to provide a confirmation code.
To receive this code please fill out an application or call our office.
Upload or email the following documents:
Copy of Birth Certificate
Current Immunization records
Physical health form ( Form DH-3040 )
Pay the Registration fee ($300 until June 15, $500 After June 15)
